How does one get to the Mall of the dead level without a warp code?

To get to Mall of the Dead, you have to throw dynamite into the water in Dark Carnival and blow up a crack, which leads to a machine thing, and then you have to input a certain combination which I can't quite remember right now. DD will certainly clarify.

cruaich nailed it perfectly.

in that level you should blow up the wall with tnt. but, it doesn't have the "crack" mark.
