Well, you will need to download Transfusion it self. I would recommend getting Beta3. You can find a link to that on our Files page. You will also need either Microsoft Windows, or some flavor of Linux, I'm not sure how far along the Mac OS-X support has come.
Hardware: You will want a 3D graphics card that supports OpenGL, something along the lines of a GeForce3 or later or one of the ATi Radion cards.
Once you've downloaded the zip archive, you'll want to extract the files keeping the directory structure and execute either Transfusion.exe for Windows or transfusion-glx for Linux

OpenGL support is essential for playing tf as darkplaces (tf engine) is not standard software quake engine. Also, as Cruaich already said, you'll need at least GeForce 3 class card (not GF 4 MX), otherwise graphics will look extremally messy. You can adjust video performance by disabling some features (mainly anisotopic filtering, rt shadows, decreasing screen resolution and disabling bloom effect) but still requirements for smooth gameplay are quite high.
