Keyboard/Mouse Setup

I've finally gotten Blood to work, the only problem is reconfiguring everything to modern fps controls. I want to use my mouse to look around/up/down and use wasd etc. I understand the controls can be changed within the setup.exe but I'd really appreciate if someone could give me a run down of their setup. I have pressed U to enable mouse aim and whatnot but I can only look around me and not above or below. Thanks.
